Application for "Out-of-State" Vehicle Permit

If vehicle is here temporarily and will be going back to the registering State, you would use this form to register.

Specimen Signature Card

If vehicle was registered without signatures on the Application for Registration of Motor Vehicle or Application for Registration of Trailer a Specimen Signature Card will be issued to each registered owner. 

Application for Duplicate Certificate of Registration

If you have lost your registration or it was stolen you would use this form to obtain a duplicate certificate of registration.

Application for Duplicate Certificate of Title

To obtain a duplicate certificate of title you will use this form;

• No lienholder on record - ALL registered owners must complete with a notary public or they must all come in person with valid government issued photo ID. 

• Lienholder on record - If there is a lienholder listed on the vehicle registration then only the lienholder can request a duplicate title. We do not accept lien release letters in lieu of the application for duplicate certificate of title signed by lienholder.

Application for Replacement of License Plate and/or Decal

A registered owner must complete the application to obtain replacement license plates and/or decal for their vehicle. Safety inspection must be valid. 

Application for Special Number Plates

For Special Number license plates you will need to complete this form and the fee is $60.00 to order.

Notice of Transfer

If you have lost or damaged the original notice of transfer from the top of your title, you may use this form. It needs to be completely filled out will all fillable spaces completed. 

*This can only be submitted if new owner was given the certificate of title.

Purchaser - Power of Attorney

This form can be used if the purchaser(s) is unavailable to sign the title. There can be no lienholder on the certificate of title and requires notarization to accept. 

Sellers - Power of Attorney

This form can be used if the seller(s) is unavailable to sign the title. There can be no lienholder on the certificate of title and requires notarization to accept. 

Bill of Sale

A bill of sale may be needed if:

• Seller(s) unable to sign section A of the Hawaii Certificate of title.
• Seller(s) unable to sign on the Out of state title in sellers section.
• MSO/MCO is not notarized in the re-assignment area.

Requires notarization. We are unable to accept if not notarized. 

VRL-MVR511 - Notice of Change of Address

Affidavit for Collection of Personal Property of the Decedent

If the registered owner for vehicle is deceased and there is no joint owner listed on the title you may complete this form if you are the successor of the deceased. This will need to be submitted with an original certified copy of the death certificate.

*You are not able to use this form if there is an appointed personal representative*

Application for "JUNK" Status

If your vehicle is non-operational and will never go back on the road you may JUNK. You will need to submit this form with the original certificate of title properly endorsed by ALL registered owners and license plates form vehicle. 

*Once you submit this form the vehicle can NEVER be registered on the roads again*

Letter of Authorization

This form is used to authorize specific personnel to sign for a company. This must be submitted at the time of registration or transfer into the company name, we will not be able to process your transaction for a company unless we have an authorization letter on file, or it is brought in at the time of registration/transfer. 

It must be completed with an affixed corporate seal or notarized. We will not accept the document without a seal or notary. 

Military Resident Form

If you are an active Military member and you are on orders in another State or Country with your vehicle you may submit this form in place of the safety inspection requirement. Must be completed and signed by the registered owner/active military member. 

Resident Form

If your vehicle is temporarily Out of State you may use this form to waive your safety inspection requirement for registration renewals. The registered owner must complete and sign. 

Statement of Fact

If your vehicle is non-operational and will be off the road you may submit this form with both license plates and last issued registration to store prior to expiration of registration. Fees will be assessed if submitted after registration has expired. 

Vehicle Location Certificate

If your vehicle is registered in Hawaii County but your mailing address on record is a different County or State, you will need to submit this form every renewal.
