Address Change By Mail

If changing address by mail, the address change will be on your record only.

To change your driver license or state identification address by mail, provide the following:

Change of Address Form - must print & sign (We do not have a system to receive these electronically)
Two (2) acceptable documents addressed to you at your new address (If mailing address differs from physical address, you must provide a document for each address) Refer to acceptable documents list.
